Care Management

Reduce gaps in care for rising-risk and high-risk patients by leveraging analytics-driven interventions and empowering care management teams with actionable insights

Supported by analytics, a robust medical management workflow, and an exhaustive rules engine, Indegene Care Manager® stratifies the population into high-risk and rising-risk groups, enrolls the targeted patient population into care management programs, develops a personalized care plan, and enables care managers to assign care coordinators who have a longitudinal view of the patient for timely care interventions. This enables providers to reduce gaps in care, optimize investments in care delivery across multiple care settings, and achieve better health outcomes.

How It Works

Gaps in care are identified using population risk stratification algorithms and subsequently personalized care plans are developed using analytics-driven insights from multiple sources of data - Claims, EMR, Pharmacy, HRA, Lab, and others.

Care plans are then assigned to the relevant care management teams across health care settings. Facilitated by seamless exchange of data among care coordinators, care managers, and administrative departments to monitor progress, Indegene's care management team effectively schedules follow-ups, addresses barriers through targeted interventions, and improves patient adherence through engagement and education. Intuitive dashboards and reports are generated to aid care managers and care coordinators to prioritize tasks and evaluate performance of teams and individuals.
